We have an exciting opportunity to welcome a Clinical Nurse Specialist, to join our Community Nurse Specialist team, based in and working throughout Taunton.
You will undertake the holistic assessment, planning and delivery of specialist palliative and supportive care for patients, with advanced, progressive life limiting disease.
This role requires working weekends and bank holidays on a rota basis as well as contributing to the out-of-hours advice line; therefore, flexibility and teamwork are crucial in providing this service.
What can you expect to be doing?
As a CNS, you will manage a caseload of patients, delivering holistic care and specialist advice to support patients and their families through complex and sensitive stages of their journey.
You’ll work closely with GPs, community teams and other professionals, acting as a key resource for palliative care across the local healthcare system.

As this role is based in the community, the ability to travel is an essential requirement.
